STUDENT RECORD REQUESTS

 Our office is closed for "Summer Shut-Down" from June 2, 2017 to July 2, 2017 and closed again on July 4th for Independence Day. However, we can still fulfill student transcript requests and education verification requests twice a week during this time.

There is no charge for this service. Turnaround time is 2 to 5 business days upon receipt of request.

(We are unable to provide copies of diplomas.)


How to Request Transcripts

Print, complete, and fax this form, along with a copy of your photo ID to 623-915-8169.

Print, complete, and email this form, along with a copy of your photo ID to MoonValleyRecords@guhsdaz.org.

MAIL

Print, complete and mail this form, along with a copy of your photo ID to:

                 3625 W. Cactus Road
                   Phoenix, AZ 85029

IN PERSON

 

 

Go to the Records Office, located in the front office of Moon Valley High School, during regular business hours and complete the form.

 *Please note that a photo ID will be required.


How to Request an Education Verification

Required information to include: 

  1. Student’s legal full name, include a maiden name or other name used if applicable
  2. Year student graduated, or last year they attended
  3. Student’s birth date
  4. For 3rd party requests, you must provide a hand written signed release from the student, with all pertinent information, giving you permission to make the request.

All education verification requests will need to be faxed to 623-915-8169.
