STUDENT RECORD REQUESTS
Our office hours are Monday through Thursday from 7:30 AM to 4:00 PM, and 7:30 AM to 3:30 PM on Fridays.
There is no charge for this service. Turnaround time is 2 to 3 business days upon receipt of request.
(We are unable to provide copies of diplomas.)
How to Request Transcripts

Go to the Records Office, located in the front office of Moon Valley High School, during regular business hours and complete the form.
*Please note that a photo ID will be required.

How to Request an Education Verification
Required information to include:
- Student’s legal full name, include a maiden name or other name used if applicable
- Year student graduated, or last year they attended
- Student’s birth date
- For 3rd party requests, you must provide a hand written signed release from the student, with all pertinent information, giving you permission to make the request.
All education verification requests will need to be faxed to 623-915-8169.
