Moon Valley Health Office – Nurse’s Office

My name is Melissa Bowden and I am the nurse here at Moon Valley High School. We are all looking forward to another great school year. In order to provide the best care possible for your student, I have listed some guidelines to help us accomplish that goal.

(1) Emergency /Health Information card needs to be completed every school year. Medication given at the nurse’s office on an as needed basis such as; Tylenol, Ibuprofen, Cough Drops and Tums, may only be given to your student if the Health Card is signed and the medications are circled yearly.

(2) Federal law requires all students be current with their immunizations to attend school. You will receive notification letters if your student is not current with their immunizations.

(3) If your student has a chronic medical condition such as; diabetes, asthma, seizure disorder, severe allergies requiring an EPI pen, or any other chronic medical condition, please contact me for required paperwork to ensure your student’s medical needs are met.

(4) Students are NOT PERMITTED to carry any medications (PRESCRIPTION OR OVER THE COUNTER) while at school. Insulin, Inhalers, and EPI-pens are the EXCEPTION, but a self-carry/self-administer form must be on file in the nurse’s office.
